Medication Requirements.
If your child has been prescribed medication to be administered at school, we have outlined our medication requirements below. If you have any questions, concerns or require further information, please don't hesitate to contact us on the above details.
Routine Medication:
If your child requires medication at a routine time (eg. Ritalin at 11am daily), written information detailing the medication, dose and time are required from your Doctor / Specialist.
Please provide us with your child's medication in a dose administration aid (blister/webster packs), the Dr's letter and the completed consent to administer medication form.
Emergency Medication:
Additional information from your Health Practioner is required if your child requires medication as an 'emergency response' (eg. Ventolin, Epipen). Your Health Practioner will need to provide you with an action Plan.
Please provide us with your child's medication with an attached pharmacy label in its original container, the Action Plan from the Dr and the completed consent to administer medication form.
Non-Emergency Medication:
Students requiring Panadol, Antihistamines, Ointments etc require a completed administer 'as-needed' medication at school form (see attached). You will also need to provide written information from your GP/Peadiatrician stating that they prescribe the 'as needed' medication and are aware your child takes it when required at school.
Please provide your child's medication with an attached pharmacy label in its original container, the Dr's letter and the completed consent to administer medication form.
If your child's dose on their medication changes, your health practitioner will need to write a letter for the school explaining the changes. To assist the school in safely administering the medication to your child, we require all routine medication to be in a dose administration aid blister/webster packs. Please call through the office and collect a consent to administer medication form.
